Bring Our Community Together

Our church has recently joined Gwinnett Medical Center’s Faith Community Network. Through this partnership we hope to build healthier communities by combining the strengths of both the hospital and the congregations affiliated with the network.

What does this mean for you? If you require treatment at any GMC facility, let them know that you are a member of the Faith Community Network and you will immediately be linked with the hospital’s Faith Community Network Navigator. The navigator will be a part of your care team for the duration of your hospital stay and will communicate with us any information you would like us to know about your progress. The navigator will stay in contact with our church network liaison. Our liaison will be activating any resources at the church that you might need once you are discharged. This could include meals, transportation assistance; help with scheduling follow up doctor’s visit and many other needs that might arise.

You can register to become a member of the Faith Community Network at no cost. Network registration forms will be available beginning January 19. Mary Cooper, our Faith Community Network Navigator, will be available on January 26, our ‘official’ registration date. Registration is ongoing.
